How Do I join the JDRM Dental Care Plan?
Our receptionist will make sure that you understand what the Dental Care Plan offers you and will answer any questions you have. All we need you to do is complete an application form which includes a Direct Debit mandate. In addition to your monthly payments, a one-off registration fee of £9.50 per person will be charged which will be added to your first Direct Debit payment. Direct Debits will be collected from your account on the 1st of each month.
lf you decide to leave the plan for any reason, we require one month’s written notice. If you decide to leave within the first 12 months, we will invoice you for any discount you have received on your dental treatment.
